Insurance and Payment Information
Many health insurance plans and employee benefit programs cover visits with a Registered Dietitian (RD) — often with no limit and at no cost to you. At ProCare Nutrition, we’re happy to file your claim for you or assist with the claims process to make things as simple as possible.
Some insurance providers may require a co-pay or limit the number of covered visits. In those cases, ProCare Nutrition can provide you with a Superbill or an itemized invoice to submit to your insurance company for possible reimbursement. Please note that reimbursement is determined by your individual policy’s guidelines.
We recommend contacting your insurance company’s member services (the number is listed on the back of your card) to verify your coverage for nutrition counseling and learn more about the reimbursement process. Unfortunately, we cannot guarantee reimbursement from your insurance provider.
Please remember that it is the patient’s responsibility to verify insurance coverage. Participation with an in-network dietitian does not guarantee that your visit(s) will be covered.
All services not covered by insurance are the patient’s financial responsibility.
Please check your coverage carefully by asking the following questions:
- Do I have insurance benefits for medical nutrition therapy or nutritional counseling? Procedure Codes are 97802/97803
- Does my plan include any nutrition counseling benefits covered under the preventative care portion of my plan?
- Is ProCare Nutrition a covered provider on my plan?
- Do I have any out of network coverage for nutrition counseling?
- Are there any limitations on which conditions are covered? (Ask if your specific medical condition or reason for seeing a dietitian is covered.)
- What is my deductible, and has it been met?
-
How many visits per year does my health insurance cover?
-
What does the coverage amount per nutrition counseling session?
-
Is prior approval/referral required from my primary care physician?
-
Is an office copay required? What is my co-insurance?

For most insurance companies, we are considered a specialist. Therefore, your specialist co-pay is applicable and is payable at the time of service. This information is often provided on the front of your actual insurance card.
